MPH ADVISING INFORMATION:
Please complete and have your faculty advisor sign by the end of the first year of the program. Once signed, please submit it to the Director of Graduate Studies for final approval.
- Anytime your plan of study changes, a new form should be signed by you, your advisor, and sent to the Director of Graduate Study for final approval.
- It is your responsibility to ensure that the final and correct plan of study is on file with the Graduate School by the end of the 3rd week of classes in the semester in which you apply for graduation.
